Practice Nurse

Guardian Medical Centre is seeking an enthusiastic, experienced and motivated Practice Nurse to join our friendly, dynamic and forward-thinking team.

We are a well-established training practice serving approximately 12,500 patients and are committed to delivering high-quality patient care within a supportive and collaborative working environment.

This is an exciting opportunity for a skilled nurse to become part of a progressive primary care team, working alongside a wide range of healthcare professionals to provide excellent clinical care to our patient population.

**Main Duties of the Role**

The successful candidate will be responsible for delivering a high standard of nursing care and supporting the management of patients with both acute and long-term health conditions.

Essential experience includes:

– Childhood immunisations and vaccination programmes
– Cervical screening

Chronic disease management, including:

– Asthma
– Diabetes
– COPD
– Administration of injections and vaccinations
– Health promotion and patient education

The successful candidate will be expected to work autonomously whilst also contributing positively as part of the wider practice team.

In addition, we benefit from a well-established Primary Care Network (PCN) team, providing extensive support from:

– GPs
– Advanced Nurse Practitioners
– Clinical Pharmacists

– First Contact Physiotherapists
– Home Visiting Paramedics
– Care Home Teams
– Care Coordinators

**About You**

We are looking for someone who:

– Is a Registered Nurse with current NMC registration.
– Has previous experience working within General Practice or Primary Care.
– Has experience in chronic disease management.
– Is competent in childhood immunisations and cervical cytology.
– Demonstrates excellent clinical, communication and interpersonal skills.
– Is able to work independently and manage their own workload effectively.
– Has a caring, compassionate and patient-centred approach.
– Works well as part of a multidisciplinary team.
– Is flexible, adaptable and committed to continuous professional development.

Contract: Permanent

Hours: 20-25 hours per week

Days: Flexible for the right candidate
