About GHC
The Gender Hormone Clinic is a nurse-led service providing gender-affirming hormone care to adults. We opened in February 2023 with around 1,200 patients and now care for close to 4,000, with 80 to 120 new patients asking to join every month.
We're growing because patients need us. You'll join a team of about 19 across Stafford, London and Exeter, led by experienced nurse prescribers and supported by a specialist multidisciplinary team. We put compassionate, evidence-informed care, patient safety and good clinical governance first.
The role
You'll guide patients through starting and staying safely on hormone treatment. It's focused, autonomous specialist work, with time built into the day for admin.
The patient pathway:
– Video history. You assess eligibility, then review the patient's GP summary and baseline bloods.
– Consent appointment. A one-hour, face-to-face appointment in clinic covering benefits, risks and alternatives.
– Prescribing. Treatment starts only after the in-person appointment. Until you qualify, a senior nurse prescriber prescribes for your patients.
– Follow-up. Six-monthly reviews for the first two years, then yearly once stable.
A typical day is about four patient appointments, alternating with admin time. Stafford nurses spend 2-3 days in clinic and the rest remote. This isn't a fully remote role.
Key responsibilities
As your specialist knowledge grows, you will:
– Run video history and eligibility assessments, taking medical, medication and family histories.
– Lead face-to-face consent appointments, including physical observations and a clear explanation of benefits, risks and alternatives.
– Review and interpret blood results, escalating anything that needs further investigation.
– Support starting and monitoring hormone therapy within our clinical protocols, working with our prescribers.
– Monitor treatment response, side effects and complications at follow-up.
– Give clear patient education, health promotion and safety-netting.
– Write accurate clinical records and letters to GPs and other professionals.
– Recognise clinical, psychological and safeguarding concerns and escalate them.
– Take part in clinical governance, audit, training and CPD.
Pay and progression
Your salary rises to £60,000 to £65,000 once you qualify as a prescriber. The salary we advertise is the salary we offer.
– Salary: £50,000 to £55,000. We fund your V300 non-medical prescribing course from month 13 of employment.
– When you qualify, you move onto the Nurse Prescriber range.
– Where you start in the range depends on your experience, and we'll explain your figure in writing.
On a 37.5-hour week, £50,000 works out at about £25.64 an hour and £55,000 at about £28.21 an hour.
Hours and benefits
You'll work 37.5 hours a week, 8.30am to 5pm Monday to Friday, with an hour for lunch. No evenings, weekends or bank holidays.
– Annual leave: 28 days plus your birthday off, plus bank holidays
– Pension: workplace pension through NEST: https://www.nestpensions.org.uk/schemeweb/nest/my-nest-pension/contributions-and-fees.html
– Company sick pay: 5 working days on a rolling 12-month basis, on top of statutory sick pay
– Private health insurance: Vitality, after probation at 6 months
– Indemnity insurance: covered by us
– Prescribing course: V300 funded by us
– Clinical training: one hour of protected training every week
Training and support
You don't need experience in gender care. We train you from day one, and you're never on your own.
– First month to six weeks: shadowing senior nurses, then running your own history appointments.
– First year: a set caseload while you learn, with no pressure to take on more.
– Around month six: you start follow-up appointments.
– Throughout: senior nurse prescribers on hand during clinic hours, weekly clinical training, and a multidisciplinary team (endocrinology, psychiatry and psychology) for complex cases.
Your induction and training cover:
– Gender-affirming hormone therapy and the medicines used
– Clinical assessment and ongoing monitoring
– Interpreting relevant blood results
– Recognising and managing complications
– Clinical protocols and escalation pathways
– Shared care and communication with GPs
– Safeguarding and clinical governance
About you
We're looking for an experienced nurse who combines sound clinical judgement with warmth, curiosity and excellent communication. You may want to build specialist expertise outside a traditional NHS role.
Essential
– Registered nurse with current NMC registration
– At least six years' post-registration experience
– Significant experience of autonomous consultations and clinical assessments
– Strong medicines management and patient safety
– Confident reviewing clinical information and knowing when to escalate
– Excellent clinical documentation and communication
– Comfortable consulting by video and working remotely part of the week
– A respectful, inclusive and non-judgemental approach to care
Desirable
– Experience in general practice, sexual health, endocrinology, women's health or reproductive health
– Experience interpreting blood results
– Experience managing patients on long-term medication
– Keen to train as a non-medical prescriber
– Experience in gender healthcare
– Experience in an independent or CQC-regulated service
