Complex Care Nurse – Care at Home

**Location: Sheffield**
**Hours: Part-Time initially, progressing to Full-Time**

We are looking for an experienced and motivated **Complex Care Nurse** to join our growing **nurse-led Care at Home service in Sheffield**.

This is an exciting opportunity to be part of a developing service providing high-quality, safe and person-centred care to individuals with complex healthcare needs in their own homes.

The role will **initially be part-time**, with hours gradually increasing as our service and client base grows, with the opportunity to progress into a **full-time position**.

The Role

As a Complex Care Nurse, you will support the safe delivery and ongoing development of complex care packages within the community. You will work closely with clients, families, care staff and other healthcare professionals to ensure excellent standards of care are maintained.

Key responsibilities may include:

– Completing clinical assessments and care plans
– Supporting and overseeing complex care packages
– Providing clinical guidance and supervision to care staff
– Delivering competency assessments and clinical training where required
– Monitoring clients’ needs and reviewing care provision
– Supporting safe, effective and person-centred care in clients’ homes
– Working collaboratively with families and healthcare professionals

What We’re Looking For

You will need to be a **qualified nurse with relevant nursing and/or complex care experience**. Experience within community care, home care or complex care services would be highly beneficial.

A **full UK driving licence and access to your own vehicle are essential**, as you will be required to travel to clients across Sheffield and surrounding areas.
